Request for Records Procedure

PLEASE NOTE: Medical Examiner cases can take a minimum of 6 to 8 weeks before completion. Due to the nature of cases reported to our office, cases may remain under investigation with a law enforcement agency. Records are only available after the case has also been closed by the investigative agency. Your request will remain on file.

All requests must be received in writing via email, mail or fax.

Please include the following information when submitting your request:

  • Decedent's name
  • Decedent's date of birth
  • Decedent's date of death
  • Your full name, your complete address
  • Contact phone number and email address
  • Your relationship to the decedent or who you represent

Our office will contact you if there is a charge for your request. Payments must be received before records are released.

Our office does not distribute death certificates. Contact your funeral home or local registry.
