ADULT GUARDIANSHIP 
Required Information

When meeting with Michael Wells of the Jackson County Counselor's Office, you will need to bring the following information--information about the person seeking to become a guardian and the respondent (the person who will be the subject of the guardianship). Failure to provide complete, accurate and legible information will result in the matter being delayed.

Use the Information Checklist to be certain you have all the required forms and information prior to your appointment.

REQUIRED INFORMATION ABOUT RESPONDENT

  1. Name

  2. Complete mailing address, including zip code

  3. Phone number

  4. Date of birth

  5. Social Security number

  6.  List of ALL family members, including their names, complete mailing addresses and telephone numbers. Also indicate their relationship to the respondent, such as "spouse," "son," "sister," etc.

  7. Current medical diagnosis

  8. Current medical letter from current treating doctor supporting guardianship (Download this Treating Physician Memo from the Jackson County Counselor's office explaining the information the doctor needs to include in his or her letter)

  9. Names, addresses and telephone numbers for the treating doctor, social worker and/or case worker.

  10. A brief history of any legal issues.

  5. Social Security number

  6. Employer's name, address and telephone number

  7. Three references (not family members) with names, complete mailing addresses and telephone numbers.

REQUIRED FORMS

These forms need to be completed and brought to your initial appointment with Michael Wells of the Jackson County Counselor's Office.

Exhibit A: Respondent's Income and Asset Information

Exhibit B: Respondent's Family Information

Exhibit C: Consent form for information about the proposed guardian (MUST BE SIGNED by proposed guardian)

Form 10194a: Part of Exhibit C, this form explains the duties and responsibilities of a guardian and conservator (MUST BE SIGNED by proposed guardian or conservator)
